In 1997 a serial rapist in Brighton attacked women, and took their shoes as a trophy. After the sixth suspected victim disappeared, presumed murdered, he stopped. 12 years on, a woman is raped at a hotel in Brighton and her shoes taken. Within a week another woman has been raped and her shoes taken, too. As the rapist strikes a third time, Roy Grace and his team are left wondering if this is a clever "copycat" offender, or whether the rapist from 1997 has started again. And what information they might be able to learn from the past to stop the offender raping - and perhaps killing- again. Part of the novel is set back in 1997/8 with Roy Grace and Sandy living at home together in the year before her mysterious disappearance. For the first time in the series we see Sandy's perspective on their relationship and marriage.

Lynn is trying to cope with life after divorce, and her bolshy teenage daughter, Caitlin diagnosed with terminal liver disease: she can only be saved by a liver transplant. She is on the transplant waiting list, but there is a world shortage and most patients die while waiting. In desperation, Lynn discovers an organ broker who can provide her with a liver but it will cost £250,000. To save her daughter she borrows from family and friends, and a few days later the organ broker says she has found a young accident victim, a perfect match for Caitlin. Grace is working on the case of 3 bodies recovered off the coast of Brighton, and the remains lead him to a Romanian trafficking street kids for the UK sex trade; some traded as organ donors. Hours before Caitlin’s operation is due to take place, Grace informs Lynn that the liver her daughter is promised will come from a healthy Romanian girl who is going to be killed. A desperate manhunt takes place to find the teenager before she is murdered. And a grim moral dilemma faces Roy. In saving the life of the Romanian girl, he is condemning Caitlin to death.
